How To Write A Treatment Plan For Substance Use In 4 Steps

Treatment planning is an important part of the therapeutic process for individuals and the families that we serve. The treatment plans you write serve as roadmaps for the clients' recovery process while in your care. When you're learning how to write a treatment plan for substance abuse, it begins with a thorough biopsychosocial assessment of the client. This assessment will include the client's family history, major life events, history of trauma, substance use history, employment, legal, medical, and financial history. It will also include how his or her substance use has impacted all of the above. Most EMR systems should have a simple biopsychosocial assessment available to use. When you write a treatment plan be sure to use these four steps:

  1. Identifying the behavioral definitions
  2. Goals
  3. Objectives
  4. Interventions

The behavioral definitions, also known as “problem statements,” are based on the information that was collected from the biopsychosocial assessment. This assessment should few brief sentences outlining your clients' condition(s) for which they are seeking treatment. Typically, the clinician will identify a few highlighted problem areas to focus on for the behavioral definitions, which often relate to the diagnosis from the biopsychosocial. 

Some examples of behavioral definitions from the Wiley Treatment Planners are: “Continued substance use despite knowledge of experiencing persistent physical, legal, financial, vocational, social, and/or relationship problems that are directly caused by the use of the substance. Demonstrates a maladaptive pattern of substance use, manifested by increased tolerance and withdrawal. Reports suspension of important social, recreational, or occupational activities because they interfere with using.”

Treatment plan goal identification

The next step in writing a treatment plan is goal identification. What does your client want to change while in treatment? Typically, a well-written substance abuse treatment plan will have two to three goals to accomplish while in treatment--unless the treatment stay is short, such as in detox facilities. Keep in mind that along with each goal, you need to write an objective that defines what the patient will do to accomplish the goal, as well as intervention, which defines what the clinician will do to help the patient complete the objective.

Goal: Improve the quality of life by maintaining ongoing abstinence from all mood-altering chemicals.

Objective: The client will learn new coping strategies for difficult emotions and identify, challenge, and replace destructive, high-risk self-talk with positive, strength-building self-talk. I will discuss this in a group setting and with my counselor within the first 30 days of treatment.

Intervention: Assign the client a homework exercise in which he/she identifies high-risk self-talk, identifies biases in the self-talk, generates alternatives, and tests through behavioral experiments. Challenge the client to share in a group setting with his or her peers. 

Goals and objectives are commonly confused with one another in the treatment planning process. A general guideline is that if you cannot actually see the client do something, then it's a goal. A good example of this is patients improving their lives. You can't see that. If you can see the client do something, such as learn new coping strategies, then it is an objective.  It is highly valuable for objectives and interventions to be linked to the “SMART” methodologies. This focuses on the qualities of being “Specific, Measureable, Attainable, Relevant, Time Sensitive.” 

Conclusion: How to write a treatment plan for substance abuse

It's important for you to remember that once the treatment plan is written it doesn’t end there. The idea is that the treatment plan is treated as a living document. It is updated regularly throughout the course of treatment as the client improves and meets target dates on his or her treatment plan.

If there’s any silver lining to the current situation, it is that treatment centers are rushing to make treatment newly accessible for those who were previously or are now unable to physically receive treatment in person.

Virtual IOPs can be very effective, and they provide your clients with flexibility and convenience, which is necessary for moments like these.

With the spread of COVID-19 and social distancing measures, receiving care in a traditional, in-person setting has become difficult and inadvisable. How To Transition

Before diving into setting up a Virtual IOP, we like to advise taking a look at your current business and organizational structure. The idea is not to create an entirely separate program, but rather simply transition the programs you already offer to a virtual format. In other words, what are some ways in which your current treatment program can be easily lifted to the web? Most likely your staff can be trained for virtual operation, but sometimes you may need to hire someone with online experience to spearhead the project.

Thanks to the FCC’s $200 million financial package helping medical treatment centers go virtual, your treatment center can receive up to $1 million in aid toward your Virtual IOP. There are some fine details to the relief package and parts of the transition will not be covered, but the following are among the covered expenses: 

● Telecommunication Services: Voice communication services for providers or clients 

● Information Services: Internet connectivity services for providers or clients, remote patient monitoring technology, patient outcomes reporting technology, video conferencing services 

● Necessary Devices/Equipment: Tablets, smart phones, remote patient monitoring equipment for patient or provider use

To transition into Virtual IOPs, some aspects of your treatment will have to change, but these changes may be small and easy to implement. Here are some questions to start thinking about while creating your Virtual IOP: 

       ● Who: How many people per session? Will you need to have individual, group, and 

family sessions? Group meetings are definitely possible and perhaps even easier since non-essential workers are staying home. 

How long: How many therapy sessions per week? How many hours of education related to their therapy? Other programs often suggest 10 or more hours a week of therapy, and most treatment lasts between 6-12 weeks. 

Treatment: Which services will you be offering online? Which of your treatments are suited for virtual delivery? You will probably be able to transition all of your treatments into online IOPs, but it’s always good to keep this in mind. 

Sponsorship: Will your clients have a sponsor who performs sobriety checks if needed, attends weekly support meetings, and verifies that your patient is following the regimen established by your center? If so, who will this person be? Some Virtual IOPs like to involve a family member or close friend, but others choose to have a staff member fulfill these tasks. 

Analytics: What kind of data would you like to collect, and how will you use it to improve treatment? Virtual IOPs can help you collect more data, information you can eventually use to create programs that serve your clients’ individualized needs.

EMR vs EHR: What is the difference? Actually, the two terms are significantly different. However, these terms are frequently confused because some in the industry use them synonymously. I'm here to help you understand how the two are different.

The easiest way I can help you know the difference is through the terms of medical and health. An EMR (electronic medical record) has a very specific focus on a patient's medical past. However, an EHR (electronic health record) looks at a much broader view of a patient's health as a whole.

EMRs are digital or electronic versions of paper records of a patient's medical history. So, if you're a clinician, you take and keep notes of your patient's medical past. This is usually diagnosis and treatment information. The federal government started pushing medical providers to use EMRs instead of paper records years ago.

There are several reasons for this:

  1. Digital records provide more data that can be tracked over time
  2. They help you as a health care professional get patients in for important preventative visits such as health screenings
  3. Improves the quality of health care and patient monitoring

EMR vs EHR: What is an EHR?

The purpose of EHRs is to have more than just the usual medical data by having a more complete view of a patient's overall health. A good EHR will have all of the information from all of the medical professionals caring for a patient. But EHRs don't stop there. They also help you (the care provider) communicate and share patient information with other providers including laboratories, specialists, etc. Essentially, EHRs help you follow patients throughout their medical journey and make it easier for you to provide better care.

This is also incredibly useful if you're the patient because it prevents you from having to coordinate hospital visits and relay information from doctor to doctor. By now you can probably see that even though the terms EMR and EHR are frequently used synonymously, they actually mean very difficult things. But they frequently work together.   5. Ongoing Maintenance Fees: There can be a lot of fees down the line including software and hardware licensing agreements, continued education of staff, IT support fees, training of new staff, and more.
